Special regulations for each ward are available inside the ward and will be made available to you separately.

Bus lines:
Route number 1 : At Azadi Terminal Station, take bus line 203 – Get off at the Meraj/Azadi terminal and after passing 7 stops, get off the bus at the Imam Hadi (AS) first stop. Then walk about 400 meters towards your destination.
Taxi and private car lines:

Axis One: Receiving proper health services is the patient's right.
The provision of health services must:
- Be worthy of human dignity and respectful of cultural and religious values and beliefs.
- Be based on honesty, fairness, courtesy, and accompanied by kindness.
- Be free from any discrimination, including ethnic, cultural, religious, type of illness, or gender.
- Be based on up-to-date scientific knowledge.
- Be based on prioritizing the patient’s interests.
- The distribution of health resources must be based on patient safety, justice, and patients' treatment priorities.
- Be based on the coordination of all aspects of care including prevention, diagnosis, treatment, and rehabilitation.
- Be accompanied by the provision of all basic and essential welfare facilities and free from imposing unnecessary pain, suffering, and limitations.
- Have special attention to the rights of vulnerable groups in society, including children, pregnant women, the elderly, psychiatric patients, prisoners, people with mental and physical disabilities, and individuals without guardians.
- Be provided in the shortest possible time and with respect for the patient’s time.
- Consider variables such as language, age, and gender of the recipients of the service.
- In essential and emergency care, be provided regardless of cost coverage. In non-emergency (elective) cases, it should be based on defined regulations.
- In essential and emergency care, if it is not possible to provide appropriate services, after providing the necessary care and required explanations, arrangements must be made for the transfer of the patient to an equipped unit.
- In the final stages of life, when the disease is irreversible and the patient’s death is imminent, services must be provided with the aim of maintaining comfort. Comfort means reducing the patient’s pain and suffering and attending to their psychological, social, spiritual, and emotional needs and those of their family during the end-of-life period. A dying patient has the right to be accompanied by a person of their choice in their final moments.
Axis Two: Information must be provided to the patient properly and sufficiently.
The content of the information must include the following:
- The provisions of the Patient Rights Charter at the time of admission.
- The predictable regulations and costs of the hospital, including medical and non-medical services, insurance rules, and the introduction of support systems at the time of admission.
- The name, responsibility, and professional rank of the medical team members responsible for care, including the doctor, nurse, and their professional relations with each other.
- Diagnostic and treatment methods, the strengths and weaknesses of each method, their possible side effects, disease diagnosis, prognosis, complications, and all information affecting the patient’s decision-making process.
- How to access the attending physician and the main members of the medical team during treatment.
- All actions that have a research nature.
- Providing the necessary training for continuing treatment.
The method of providing information must be as follows:
- Information must be provided at an appropriate time and in proportion to the patient's condition, including anxiety and pain, and their individual characteristics such as language, education, and comprehension, unless:
- The delay in starting treatment due to providing the above information causes harm to the patient. (In this case, the transfer of information must be done at the first appropriate time after the necessary action.)
- The patient, despite being informed of the right to receive information, refuses it. In this case, the patient’s wish must be respected unless the lack of information puts the patient or others at serious risk.
- The patient can access all the information recorded in their medical file, receive a copy of it, and request the correction of errors recorded in it.
Axis Three: The patient's right to choose and make free decisions in receiving health services must be respected.
The scope of choice and decision-making includes the following:
- The choice of the attending physician and the health service provider within the framework of regulations.
- Selecting and consulting a second doctor as a consultant.
- Participation or non-participation in any research with assurance that their decision will not affect the continuation and manner of receiving health services.
- Accepting or rejecting proposed treatments after being informed of the possible consequences of accepting or refusing them, except in cases of suicide or when refusing treatment puts another person at serious risk.
- The patient’s prior statement of opinion about future treatment actions when they have decision-making capacity must be recorded and considered as a guide for medical actions when they lack decision-making capacity, in compliance with legal regulations by health service providers and the substitute decision-maker.
- – The conditions of choice and decision-making include the following:
- The patient's choice and decision must be free and informed, based on receiving sufficient and comprehensive information (mentioned in the second axis).
- After providing information, sufficient and appropriate time must be given to the patient to decide and choose.
Axis Four: The provision of health services must be based on respect for the patient's privacy (the right to solitude) and the principle of confidentiality.
- Respecting the principle of confidentiality regarding all patient-related information is mandatory unless the law provides an exception.
- At all stages of care, including diagnosis and treatment, the patient’s privacy must be respected. Therefore, all necessary facilities must be provided to ensure the patient’s privacy.
- Only the patient, the treatment team, individuals authorized by the patient, and those permitted by law can access the information.
- The patient has the right to have a trusted person accompany them during diagnostic stages, including examinations. The presence of one parent alongside a child at all stages of treatment is the child’s right unless this contradicts medical necessity.
Axis Five: Access to an efficient complaints handling system is the patient’s right.
- Every patient has the right, in case of claiming a violation of their rights as stated in this charter, to file a complaint with the competent authorities without any disruption in the quality of receiving health services.
- Patients have the right to be informed about the handling process and the results of their complaint.
- Damages caused by the error of health service providers must be compensated according to regulations after review and confirmation, in the shortest possible time.
- Baby Birth Guide
- In order to obtain a birth certificate for a newborn, the parents' ID cards must be submitted to the delivery block (maternity ward).
- If you have any problems or questions about breastfeeding, you can visit the neonatal clinic located on the ground floor (polyclinic) every day from 5:00 PM to 3:00 PM.
- To vaccinate your baby, visit the clinic's vaccination room on Saturday and Tuesday mornings.
- The free training class for pregnant mothers is being held at the hospital. Each course is two months long and there is one session per week. At the end, you will receive an educational booklet and a visit to the delivery block.
- Patient Accompanying Rules
If a companion is needed, a companion card must be provided upon admission.
One companion is allowed to accompany the patient in the ward, but no more than one person is allowed.
Changing the patient's companion is allowed until 12:30 PM, otherwise it is not possible.
The hospital will only provide lunch and dinner for one companion. If you need additional food, you can go to the restaurant on the 1st floor to prepare chips and eat.
There is a sofa in the room for companions, which can be converted into a bed by pulling the relevant latches.
- Patient Visitation Rules
The hospital's visiting hours are daily from 2:30 PM to 6:00 PM.
The visiting hours in the special wards (CCU - POST PCI) are daily from 3:00 PM to 4:00 PM. And the POST ICU is daily from 3:00 PM to 5:30 PM. The number of visitors to the special wards is limited. Visits to the special wards must be coordinated with the Takrim Unit personnel.
Children under 12 years of age are prohibited from entering the hospital.
Bringing natural flowers with roots to the hospital is prohibited.
To enter the hospital from 7:30 PM to 6:00 AM, you can only enter through the emergency and nuclear medicine doors.

It is essential that online appointments are made only with the patient's details and information (any changes, transfers or refunds are not possible).
Currently, online appointments are not possible for foreign nationals and newborns who do not have a national ID.
When registering an online appointment, 25% of the free visit fee will be deducted from your account, and the remaining amount will be charged for the time you are in the hospital.
If you have insurance, it is mandatory to present the patient's insurance book and if you do not have insurance, it is mandatory to present the patient's national card to the hospital fund.
Please check your appointment again 24 hours before visiting the hospital after registering for an online appointment at Razavi Hospital.
If the doctor is not present at the clinic at the scheduled time, you will be notified via SMS and you can directly contact the hospital clinic fund to get a refund for the visit fee.
If you do not show up at the scheduled time for which you have made an appointment, your appointment will be canceled and your payment will not be refunded.It should be noted that online appointment scheduling is not possible from 11:15 PM to 12:15 AM.
